Case Plans

How to create a case plan, build out goals and actions, generate suggestions from an assessment, and review or export a plan.

A plan tracks the goals and actions you're working on with someone, with a review history and AI-assisted drafting from an assessment. Case plans need to be switched on for your organisation, and follow the same access as the case itself.


Creating a plan

On a case's page, click Create plan - or View plan if one already exists, since a case only has one. This opens the plan editor.

A read-only summary of the plan (goals, actions, case worker, review due date) also appears directly on the case page, with a View full plan link into the editor.


Building the plan

Click Add goal to add a goal, then give it a title and a Description. Under each goal, click Add action to add a row with Assigned to, Target date, and Status (Not started, In progress, Done, or No longer relevant) - overdue actions are flagged automatically.

Add evidence on an action lets you link an interaction, upload a document, or add a note to show the work behind it.


Generating suggestions from an assessment

Click Generate from assessment and pick one of the case's assessments. The AI drafts new goals and actions from it, shown as Suggested goals and Suggested actions cards, each with the source quote from the assessment and a confidence level.

Click Add to plan to accept a suggestion, or Dismiss to skip it - an accepted suggestion can be undone afterwards. If the plan was empty, suggestions are added automatically as an editable first draft.


Reviewing and exporting

Click Mark as reviewed to save a permanent snapshot of the plan - the plan itself stays open for further edits. Past reviews are listed under Review history, each showing who reviewed it and when.


Use Export to download the current plan, or any past review, as a Word document or PDF.
